2026-2027 DIAA ATHLETIC PHYSICAL AND CONSENT FORMS
Any athlete interested in taking part in any tryouts or sporting events, a 2026/2027 DIAA physical examination form signed by a licensed doctor (MD or DO), licensed nurse practitioner, or licensed physician's assistant that was done on or after April 1, 2026, must be on file with Nurse Rebecca or the McCullough Wellness Center.

No DIAA physical on file means the student-athlete will not be able to participate in tryouts or sports.

McCullough Athletics 2026-2027 Home Game Expectations
Student-Spectator Expectations:
-
All student spectators must be accompanied by a verifiable parent/guardian in order to attend athletic events.

Events begin at 3:30 p.m. All student spectators must leave school grounds at the end of the day and return with their parent/guardian at 3:25 p.m.

Spectators are not provided with transportation to or from athletic events.

There will be a concession stand of snacks and drinks for the game.

The Colonial School District Code of Conduct is in effect at all games.
Sportsmanship Expectations:
McCullough spectators should…

be positive and recognize strong effort, skillful play, and teamwork

be respectful and supportive of our student-athletes, our opposition, coaches, officials, and other fans

be good-natured and sportsmanlike

appreciate that officials are providing a service that allows our student-athletes to compete; No Officials, No Competitions

understand that middle school sports are about increasing student skills and knowledge of their sports in preparation for high school athletics

recognize and appreciate the importance of team sports in youth development and character-building

recognize that there is a way to win with class and a way to lose without excuses or finger-pointing
Booing or taunting an opposing player, fan, or game official is unacceptable at every McCullough athletic event. Cheers, chants, or signs or gestures that are obscene, mocking, or derogatory are not reflective of our core values and will not be tolerated. Such negative behavior may lead to prohibition from future attendance at athletic events.

DIAA Winter Sports - Boys Basketball, Girls Basketball, Wrestling, Cheerleading
DIAA Spring Sports - Baseball, Softball, Track and Field
2.5.2 In order to be eligible for participation in interscholastic athletic contests and scrimmages, a student must pursue a regular course of study or its equivalent as approved by the local governing body and must be passing at least four full-year courses.
